I confirm that I am a student currently enrolled as an undergraduate in a Commonwealth Supported place at The University of Adelaide, University of South Australia (UniSA) or Flinders University studying an eligible discipline.

I understand that providing any false or misleading information may prohibit me from participating in the Defence Industry Connection Program and that any monies received by me may be recovered by the program's administrator.

I am eligible to apply to secure a baseline clearance through the Australian Government Security Vetting Agency (AGSVA) where required by the host company.

I understand that I may be providing health information which is sensitive information under the Privacy Act 1988 (Cth);

I consent to Australian Industry Group's collection, use and disclosure of the information for the purpose stated in this form; and
